A Clinical Study on Peptic Ulcer Perforation




Abstract
This report is a clinical analysis of 59 patients of perforated peptic ulcer who were treated at the department of General Surgery in the Chung-Ang University hospital from January 1981 to December 1985.
And the results were as follow:
1) Among these 59 cases, the perforated gastric ulcers were in 13 cases(22.0%) and the perforated duodenal ulcers were in 46 cases(78.0%).
2) In age distribution, the highest incidence was 3 rd decade, and 5th decade was followed.
3) The sex distribution was 57 in male and 2 in female with a ratio of 28.5: 1,
4) The monthly distribution was high in May, low in July and September.
5) The duration of characteristic peptic ulcer symptom prior to perforation was most common for under by 1 year by 17 cases(28.8%), but 11 cases(18.6%) had no previous ulcer symptom.
6) By the physical findings on admission, the majority of cases had abdominal tenderness (93.2¡Æ0), abdominal rigidity(88. 1%), rebound tenderness (89.9%).
7) Among the 4 types of the blood group, 0 type was relatively common than others.
8) The subdiaphragmatic free air shadow was seen in 45 cases(76.3%) and the preoperative duration after perforation was within 12 hours in 24 cases(40.6%).
9) The most frequent location of perforation was in the anterior wall of the duodenal first portion with 41 cases(69. 5%) and the size of perforated orifice¢¥ was less than 5 mm in diameter with 44 cases (74. 6%).
10) As the operative procedure, subtotal gastrectomy in 30 cases(50.8%), truncal vagotomy with pyloroplasty in 11 cases(18.6%), truncal vagotomy with antrectomy or hemigastrectomy in 4 cases(6.8%), simple closure in 14 cases(23.8%), were performed.
11) The early postoperative complications were noted in 12 cases(20.3%) and the common complication was wound infection in 4 cases(6.8%).
12) The mortality rate was 1.7% (1 case) and the cause was cardiorespiratory failure.
